READING OR DOWNLOADING

We collect and store only the following information about you: the name of the domain from which you access the Internet (for example, aol.com, if you are connecting from an America Online account, or princeton.edu if you are connecting from Princeton University’s domain), the date and time you access our site, and the Internet address of the Web site from which you linked to our site.  We use the information we collect to measure the number of visitors to the different sections of our site, and to help us make our site more useful to visitors.

ONLINE PROFILE UPDATES

If you complete the Profile update form and share your personally identifying information, this information will be used only to provide you with more targeted content. We may use your contact information to send further information about our organization or to contact you when necessary. SENDING US AN EMAIL

We may forward your email to other Foundation staff members, staff at the American Physical Therapy Association “APTA”, board members, volunteers, or consultants, who are better able to answer your questions. MAKING A DONATION OR REGISTERING FOR AN EVENT

Should you choose to make a donation or register for an event online, we will collect your payment information (credit card type, credit card number, credit card expiration date, and security code) in addition to your billing information (name, organization [if applicable], and complete address) and any other information necessary to complete the transaction. Upon transmittal of your data to process your credit card, the credit card information becomes encrypted, secured, and no longer available to staff members. Non credit-card information related to the donation will be secured in your profile in our database.

The Foundation for Physical Therapy Research is a charitable, 501(c)(3) not for profit organization. EIN: 13-611225. All donations are tax-deductible to the fullest extent of the law. Electronic receipts will be sent for each donation received. In any instance in which a full tax-deduction cannot be given, an explanation of benefits received and their corresponding amount will be included in the tax-receipt letter. Should you not receive your electronic receipt, or if you require a hard copy, please email us at info@foundation4pt.org.

The Foundation for Physical Therapy Research will honor any request for a donation to be designated to a specific fund or activity or for a donation to remain anonymous.
